Our Current Projects

The Community Health Worker (CHW) Initiative

Launched in September 2024, CCHI’s Community Health Worker (CHW) Program is designed to strengthen the local health workforce and expand access to care across California.

In partnership with Partnership HealthPlan of California, we train and certify CHWs with the skills and tools they need to support their communities.

Through collaboration with our longtime member organization, California Community Health Initiative (CalCHI), we also provide CHW services and help our partner bill for these essential services.

Centers of Medicare & Medicaid Services (CMS) 2022 Healthy Kids Grant

Since October 2022, CCHI has worked with 6 amazing subcontractors in 7 California counties under the CMS 2022 Healthy Kids Grant.

This initiative focuses on providing Outreach, Enrollment, Retention, and Utilization (OERU) services for children, parents, and pregnant individuals to enroll into Medi-Cal, or the Children’s Health Insurance Program (CHIP).

Our Past Projects

Medi-Cal for Older Adults (MFOA)

Previously, CCHI contracted with the Department of Health Care Services (DHCS) and the California State Association of Counties (CSAC) to provide Outreach, Enrollment, Retention, and Utilization (OERU) support to older adults and seniors facing barriers to health access.

From October 2022 through June 2024, CCHI partnered with 10 organizations in 11 California counties to ensure better health outcomes for the target populations.

SB-154 Navigators Grant

Beginning in November 2022, CCHI contracted with the Department of Health Care Services (DHCS) under the SB - 154 Navigators Grant.

Under this project, CCHI worked with 9 organizations in 9 California counties to provide Outreach, Enrollment, Retention, and Utilization (OERU) support to individuals in specified target populations, including: individuals with limited English proficiency, families with mixed-immigration status, individuals experiencing substance abuse disorder, and individuals experiencing housing or food insecurity.

Advocacy & Policy

Annually, CCHI brings together our member organizations and health advocates across the state to elevate their community’s voices at our Day at the Capitol.

Through legislative visits and coalition-building, CCHI and its members advocate for policies that protect and expand access to health care for all.

Our advocacy efforts are grounded in lived experience and driven by the belief that the communities most impacted by health inequities should lead the conversation for change.

Health Education

CCHI is committed to keeping our members informed, engaged, and equipped to support their communities.

Through Health Access Assister (HAA) Forums, statewide membership meetings, and timely resource sharing, we provide education on health policy, program updates, and best practices in enrollment and outreach.

Our educational initiatives ensure that front-line staff and organizational leaders alike have the tools they need to navigate California’s evolving health care landscape.
